    The purpose of this form is to provide us with current information on School-Business Partnerships. Please provide the necessary information for your partnership.  We ask that you print and fax the completed form in to 337.217.4132.

•  Activities Report Form

The purpose of this form is to provide us with a summary of quality activities conducted by the business and/or by the school.  The information you provide will be used to update the PIE Projects page. Once the form is completed, please select the submit button to email it to the PIE coordinator. You will receive a confirmation that your email has been sent. It is recommended that you print this page for your records.

Schools may also submit activities through the PIE blackboard. School liaisons have been enrolled and may post activities in the Discussion board area. The activities will be posted to the PIE Projects page of the website.
