McNeese State University United Way - Success by Six

Guidelines

  1. Must be three (3) years old or four (4) years old on or before September 30, 2008.

  2. Must have child’s certified birth certificate.

  3. Must have child’s Social Security Card.

  4. Must have child’s current Shot Record from the Health Unit or physician.

  5. Must have Blue WIC Folder

  6. Must have a copy of Insurance Card or Medical Card

  7. Must have a copy of Food Stamp Card and Eligibility Letter

  8. Must have Proof of Income (TANF, Child Support,  W-2, SSI Income or your Check Stub)
