TUITION EXEMPTION INFORMATION

· Teacher Tuition Exemption Application

· Applications will be accepted on following dates for each semester:

                        Summer, 2008             May 5, 2008 – May 30, 2008

                        Fall, 2008                    August 18, 2008 – September 15, 2008

Spring, 2009                TBA

· Listed are the priorities for receiving tuition exemption:

Priority # 1: Non-Standard Certification (TAT, TEP, EP)

Enrolled in Alternative Certification Program

Teaching Out of Field (OFAT)

Priority # 2: Certified-Seeking Highly Qualified

Priority # 3: School Improvement as a Cohort

Priority # 4: School/Teacher Leader

· All applications along with all attachments should be submitted to Bethany Bond in the Personnel Department located at 1724 Kirkman Street, Lake Charles, LA 70601.

Attachments are as follows:
-Copy of Teaching Certificate

-Paid Fee Bill

-Prescription (courses required to complete field of study) signed by the department head or advisor at the university attended.

At completion of the semester:

-Original transcript from university (If mailed, transcript must be sent to the attention of Bethany Bond).
