19-20 School Supply List
Registration Fee - $30.00 includes a $25.00 school assessment and postage fee and a $5.00 locker maintenance fee.
All students will be required to purchase a Physical Education uniform either at a local vendor or purchase one designed specifically for Oak Park Middle School in the school colors - purchased at school at same cost as vendors ($22.00 each) available for purchase at registration.
Additional supplies may be needed during the school year to replenish supplies as used
